What is a SIBO Test?

SIBO tests, most commonly performed as breath tests, are used to detect small intestinal bacterial overgrowth (SIBO) as well as malabsorption of sugars like lactose or fructose. Malabsorption occurs when the small intestine does not fully absorb certain food components, leading to digestive issues. These conditions can produce symptoms similar to acid reflux and may occur on their own or alongside GORD (Gastro-Oesophageal Reflux Disease). Identifying SIBO or malabsorption is important because, if left undiagnosed, patients may undergo unnecessary surgery—or, if surgery is performed, it may be less effective or cause side effects. For example, a patient with bloating caused by SIBO may find that a Nissen fundoplication procedure worsens this symptom rather than relieving it.

What causes SIBO?

SIBO typically develops when the normal movement of food and bacteria through the digestive system is disrupted, creating an environment where bacteria can overgrow in the small intestine. Common causes include:

  • Motility disorders: Conditions that slow down the movement of food through the digestive system, such as irritable bowel syndrome (IBS) or nerve damage related to diabetes.
  • Structural issues: Problems like bowel surgery, scarring, or adhesions that disrupt the normal flow of food.
  • Immune system conditions: Health issues that affect immunity can alter the balance of gut bacteria.
  • Low stomach acid: Reduced stomach acid can decrease the stomach’s ability to kill bacteria before they reach the small intestine.
  • Medications: Long-term use of proton pump inhibitors (PPIs) or antibiotics can disrupt healthy gut flora.

To diagnose SIBO, you may undergo a hydrogen or methane breath test, where you drink a sugar solution and your breath is analysed for gases produced by bacteria in the small intestine. In some cases, a small fluid sample may be taken from your small intestine during an upper GI endoscopy to check bacterial levels, and blood tests can be done to assess nutritional deficiencies or inflammation.
